Syndrome:  Pyelonephritis, uncomplicated

Primary Therapy(s):

  • Ciprofloxacin x 5-7 days

Alternate Therapy(s):

  • TMP/sulfa x14d
  • Augmentin x 14d
  • Ceftriaxone (IV)

Comments:

  • This applies to patients without complicating urologic problems.
  • P.O. beta-lactams not as effective as FQ's.
  • There now exists 10-20% FQ and TMP/SMX resistance in E. Coli. An empiricial dose of cefriaxone or gentamicin might be considered pending culture results.
