Although review of this patient's lymph node pathology did not reveal well defined granulomas, the intense mononuclear
inflammatory reaction to Mycobacterium avium complex infection that was present was likely responsible for elaboration of
excessive 1,25-dihydroxyvitamin D3 causing the observed hypercalcemia.
